Anxiety Self- Assessment
A brief screening tool designed to help individuals better understand anxiety-related symptoms and patterns.
This assessment is intended for educational purposes only and is not a diagnosis or substitute for professional medical advice.

Anxiety can affect individuals in many different ways, including persistent worry, nervous system overwhelm, racing thoughts, sleep disruption, irritability, difficulty relaxing, or ongoing stress that feels difficult to regulate.
This self-assessment is based on a clinically recognized screening framework commonly used to better understand symptom frequency and severity.
Understanding Your Score
0–4 → Minimal symptoms
5–9 → Mild symptoms
10–14 → Moderate symptoms
15–21 → More significant symptoms
This screening is not a diagnosis. If your score is 10 or above professional evaluation is recommended.
You do not need to navigate this alone.
If anxiety symptoms have been persistent, overwhelming, or difficult to manage, professional support may help you better understand available options and next steps.
At Z&Z Wellness, we approach care through thoughtful evaluation, individualized treatment planning, and medically guided support.
